To request a gynaecology consultation, please complete the short form below with as much information as possible about your symptoms.

Common gynaecology symptoms may include irregular bleeding, period problems, pelvic pain, menopausal symptoms, or other women’s health concerns. This list is not exhaustive.

Please note: This service is intended for gynaecological symptoms or concerns that have been present for 5 days or more. If your symptoms have been present for less than 5 days, please arrange a routine appointment with a clinician instead.

Breast symptoms or concerns are not suitable for this form. If you have a breast lump, breast pain, changes to the breast or nipple, or any other breast concern, please book a routine appointment with a clinician.

This service is also not suitable for contraception discussions or requests. Contraception can be discussed during a routine on-the-day appointment with any clinician.

All forms are reviewed and triaged by a clinician. You will be contacted within 5 working days with the outcome of your review. Following triage, you may be sent a link to book either a face-to-face or telephone appointment, depending on what is most appropriate for your symptoms. In some cases, you may be directed to a more suitable service.

If any of the following applies to you, please do not complete this form and instead contact the practice directly or call NHS 111 for more urgent advice:

  • You are pregnant or have recently been pregnant

  • You are acutely unwell, have a temperature, or are feeling faint

  • You have recently had a gynaecological procedure or surgery

  • You have severe or rapidly worsening symptoms
